CYP2C19 activates Clopidogrel into the form that prevents platelets from clumping. Poor CYP2C19 metabolism weakens that protection; CPIC provides alternative-antiplatelet context after certain heart attacks or stents.

What does “intermediate metabolizer” mean in this sample?
DR. PRIME
CYP2C19 helps the liver make an enzyme that turns Clopidogrel on. Clopidogrel is an anti-clotting medicine often used after a heart attack or stent. In this sample, that enzyme works more slowly, so the body may activate less of the medicine and it may not prevent clots as reliably.
For people with this result who need Clopidogrel after certain heart attacks or stents, the 2022 CPIC guideline recommends a different anti-clotting medicine when suitable. It names Prasugrel and Ticagrelor as options. This gene result explains why Clopidogrel may be weaker; it cannot tell us which alternative is safest for one person.
